2021 Informed Consent Form


Attention all 2021 Wrightwood Little League parents, please be sure to sign this COVID-19 Player Informed Consent Waiver before evaluations tomorrow. 

All players must have one on file in order to enter the field.

Options to turn it in: 

1.  Please print it out and bring it to the field on Saturday. 
2.  Give to your coach or team mom once teams are assigned.
